Panama Maritime Authorities say incorrect scale charts and poor seamanship were probable case of MV Wakashio grounding.
Very interesting article in Maritime Executive related to the grounding of MC Wakashio on Mauritius.
The vessel was registered in Panama so their Maritime Authorities have had their own team in Mauritius conducting an investigation into events leading up to the grounding.
It does not sound as if the Captain and his Navigation team conducted themselves professionally.
The suggest that the reason for the proximity to the coast was to get a good phone signal so a crew member could phone home on his birthday, the wrong scale chart was in use, calls from the Mauritius authorities went unanswered, the Captain, Chief Engineer and First officer were on the bridge and at one stage the navigation system was lost.
The report goes on to cite, “a lack of supervision and monitoring of the navigation equipment, the distraction generated when the officer of the watch totally loses the navigation course, and an excess of confidence during the watch.”
The report concludes that “with a safeguard and applying good seamanship practices, an appropriate analysis of the situation would have allowed taking the pertinent actions to correct the course and avoid the accident.”
To date the meriting Authorities have not been able to interview the Captain.
